The Healthcare for the Homeless program is more than just health care. We believe in treating all the needs of an individual, including underlying issues that cause or continue homelessness. This holistic approach fosters an atmosphere of respect and provides hope to hundreds of individuals every year. Our dedicated case management team provides support, guidance and resources to help individuals end their homelessness, as well as manage their medical care. We are partners in your care, preparing our clients to be “housing-ready” and collaborate with partner sites that serve veterans, youth and families. Our staff can help break the cycle of substance abuse and dependent behavior among individuals who are homeless. Using motivational interviewing techniques and a harm-reduction model, our staff provides assessment and individual counseling. We are also experts on community resources, including our outpatient program, Solutions for Recovery. A trained Substance Abuse Counselor is available at our Downtown Family Health Center at Connections.

A dedicated team engages individuals in communities across San Diego to build relationships and trust, guiding and encouraging them to access resources to end their homelessness. This team also works collaboratively with other homeless service providers and provides education to groups and service providers about the comprehensive services available at FHCSD to individuals experiencing homelessness.

Health Care for the Homeless (in English or Spanish) support groups are a safe place for individuals experiencing homelessness to share resources, discuss emotions and find hope within each other. We can also provide Case Managers and Peer Support Specialists for participants.
