For more than 55 years, Family Health Centers of San Diego’s mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons.
Family Health Centers of San Diego (FHCSD) is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, two PACE centers and a pharmacy.
Our staff provides care to more than 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. Services include adult and pediatric care, chronic disease management, comprehensive women’s care, including obstetrics, dental and vision services, case management, physical rehabilitation, children’s speech therapy, vaccinations, infectious disease treatment, behavioral health, substance use counseling and specialty care such as cardiology, podiatry, endocrinology and dermatology. We also offer intensive case management for unsheltered individuals.
The expansion of our clinic locations, services and programs has made us the largest community clinic provider of health care to the uninsured in San Diego County and one of the 10 largest community clinic organizations in the nation. We also serve as the region’s leading safety-net, school-based and mental health care provider.

Patient-Centered Medical Home
Family Health Centers of San Diego (FHCSD) is an accredited Patient-Centered Medical Home (PCMH) by The Joint Commission and the National Committee for Quality Assurance (NCQA), organizations dedicated to enhancing health care quality and safety.
At a PCMH, patients receive quality care through a direct and long-term relationship with their chosen provider and health care team. This team is respectful of and responsive to individual patient preferences, needs and values, ensuring all clinical decisions are guided by these principles. Patients, and their families, also receive support to become engaged in healthy behaviors.
Everyone at FHCSD — from clinicians to receptionists — plays a role in creating a PCMH that supports whole-person care and improves access, self-care and care coordination within and outside FHCSD.
