1. Call (619) 515-2559 to register. Provide your name, date of birth and personal email address to the patient portal technician.
2. Once we have this information, check your email for a welcome message with instructions to access the website. You will be prompted to enter your date of birth and complete the registration process. If we already have your email on file, you can also self-register or log in directly at ppm.fhcsd.org.

If you are an existing patient and have been seen by a provider in the last three years, you can schedule general appointments with your provider.
