San Diego, CA — Family Health Centers of San Diego (FHCSD) today announced a grant of $250,000 for Health and Well-Being Innovations made possible through the Prebys Foundation’s over $4 million investment in community health across San Diego County. The funding will help FHCSD expand and enhance its Street Medicine Team (SMT), which delivers medical and behavioral healthcare directly to individuals experiencing unsheltered homelessness throughout San Diego. The grant will support the purchase of vital equipment, mobile medical supplies, and outreach resources, enabling the SMT to reach more people in need and provide comprehensive care in non-traditional settings.

“This incredible support helps us continue to serve vulnerable San Diegans who depend on us for essential care,” said Meredith Johnston, Vice President Development. “We’re committed to ensuring that everyone in our community — regardless of income or background — can access the care they deserve.”

The Prebys Foundation’s recent grants were awarded to 23 trusted community clinics and health organizations across San Diego County — all dedicated to expanding access to healthcare and addressing health equity challenges.

FHCSD will use this funding to increase the capacity of its Street Medicine Team, allowing for more frequent outreach visits, expanded behavioral health services, and improved connections to housing and social services for people living unsheltered.

This effort is part of United for San Diego, a collective initiative by the San Diego Foundation, Price Philanthropies, and the Prebys Foundation that mobilizes regional partners around a shared goal: protecting access to food, housing, and healthcare for all.
