FHCSD offers free case management services through our enhanced care management (ECM) program for eligible Medi-Cal patients. Our case managers work closely with your medical providers, pharmacists and other specialists to support your overall health and well-being.

- Connect with health care providers & schedule your appointments
- Develop a personalized care plan
- Understand your medications & treatment plan
- Plan your medical, dental & mental health visits
- Set up transportation for appointments
- Coordinate your care after a hospital stay
- Access essential resources, including food & housing

Who is eligible for ECM?
+To qualify for ECM, you must be enrolled in a Medi-Cal health plan and meet one or more of the following criteria:
You must:
- Not have a safe place to live or be unhoused
- Have visited the emergency room or hospital five or more times (adults) or three or more times (youth age 21 and under) in the last six months
- Have a mental health condition, substance use disorder or behavioral health need
- Be an adult (age 21 or older) at risk of entering a nursing home or other long-term care facility
- Be living in a nursing home or facility and want to return to the community
- Be enrolled in the California Children’s Services (CCS) program and need extra support
- Have been in foster care (up to age 26)
- Have been released from jail, prison or a youth detention facility in the past 12 months and have a health condition
- Be pregnant or have had a baby in the past 12 months
