Our Social Services Clinic Integration (SSCI) program offers no-cost case management services to help you navigate our health care system.
Social services coordinators are available to link you to community and FHCSD resources, help you develop goals and work with you to overcome barriers to improving your health.
Eligibility for other case management programs will be explored before engagement with SSCI to ensure best fit.

SSCI focuses on:
- Complex medical needs or other social factors impacting your health
- Reduced or no access to adequate food
- Safety concerns
- Needing multiple social services at once
- Screening or verification of eligibility for an existing internal or external case management program
