What records can be requested

A patient or their legal representative may review or obtain copies of medical records or request that records be sent to another health care provider or facility.

Family Health Centers of San Diego requires a completed and signed Authorization to Release Medical Information form before releasing medical records, including to the patient. In certain cases, approval from a physician, psychologist or social worker may also be required.

Please specify which components of your medical record you would like to receive.

Additional authorization required
Additional authorization is required for requests involving any of the following information:

  • Psychiatric care
  • AIDS/HIV
  • Substance use
  • Fertility treatment

Processing time, fees and identification

Processing time
Please allow seven to 10 calendar days for processing. If you requested records for personal use, the Health Information Management (HIM) department will contact you when your records are ready.

Fees

  • Paper copies: $5 for five pages or fewer; $5 plus 20 cents per page for six or more pages
  • Electronic copies (CD): $15

Identification requirements
A valid government-issued photo ID is required. If someone other than the patient is picking up records, they must present the patient’s original signed authorization form and a valid photo ID.

MyHealthRecord for teens

Our online patient portal is available for patients under age 18. The secure portal provides 24-hour access to portions of a child’s medical record and makes it easier for families to communicate with their care team. The portal is free to use and accessible from anywhere.
