Family Health Centers of San Diego offers annual physicals and Initial Health Assessments (IHA)!
A physical exam is a wellness check that lets you create a relationship with your medical provider, address any health issues and talk about preventative measures for your future. Going in for a physical when you aren’t sick gives your provider a better understanding your specific health and wellness goals and can improve care in the long-term.

Why do you need an IHA?
The Department of Health Care Services requires all new Medi-Cal members complete a comprehensive IHA after enrollment. The IHA includes:
- Physical, social or mental health histories
- Preventive care services
- Physical exam
- Completion of the required forms
