Family Health Centers of San Diego Patient Portal for Kids & Teens

Teens ages 12–17 can create and manage their own MyHealthRecord portal account. The secure online portal provides 24-hour access to portions of your medical record, lets you communicate with your care team, and allows you to view, print, and save important documents. The portal is free to use and accessible from anywhere.

What you can do

With your teen portal account, you can:

  • View future appointments and past visits
  • Access lab results
  • See medications, immunizations and allergies
  • Request medication refills
  • Review your health history
  • Check vital signs, weight and BMI
  • Print immunization records
  • View, print and save documents ordered by providers, including school or work excuse notes
  • Send messages to your provider and care team

For parents of teens

Under California Minor Consent and Confidentiality Laws, parents cannot access their teen’s portal account. These laws protect minors who may seek certain services without parental consent, including:

  • Sexual and reproductive health services
  • Testing and treatment for sexually transmitted infections including HIV/AIDS
  • Pregnancy-related services and abortion care
  • Sexual assault services
  • Mental health treatment
  • Substance use treatment

Proxy access

Parents, family members or legal guardians can request proxy access for teens ages 12–17. This allows them to view and manage a teen’s health information in the portal with the teen’s signed authorization form.
