Patient Portal for Kids & Teens

patient portal for teens

Our online patient portal, MyHealthRecord, is now available for individuals under the age of 18. The patient portal is a secure website that allows patients 24-hour access to portions of their medical record. The patient portal allows you to communicate more easily with your medical provider, is available anywhere and free to use.

For Ages 0-11

If you have a child between ages 0-11, you can sign up for a child account that will allow you to view:

  • After visit summaries
  • Allergies, medications, immunizations
  • Health Summary (Problem List)
  • Future appointments and past visits
  • Lab results
  • Vital signs, weight and BMI

Parents can also print immunization records from the patient portal. To create an account for your child, you’ll need to fill out a form. Ask the front desk staff for help at your child’s next appointment!

For Ages 12-17

If you are between ages 12-17, you can sign up and manage your own account. The account will allow you to view:

  • Allergies
  • Immunizations

You will also be able to print your immunization records. Your parent will still be able to make appointments for you by calling (619) 515-2300, but won’t have access to the account.

To sign up for an account, ask your medical provider for help at your next appointment!
