What services does the WISEWOMAN program offer?
- Individual health coaching sessions to improve cardiovascular health*
- Free screenings for heart disease and stroke
- Referrals to community resources
You are eligible to join if you:
- Are between the ages of 35 and 64
- Are currently enrolled in Every Woman Counts (EWC) program
- Are due for a well-woman exam (PAP and/or breast exam)
Participants may qualify to receive a $50 food voucher.
The WISEWOMAN program promotes healthy behaviors to reduce the risk of heart disease and stroke.
